On or about November 15, 2011, at Village Creek SP GU #2 well site,
employees were
required to use a wire rope for Geronimo line that at the end section of
the line was
obstructed by a 13 3/8 J-55 pipe. This condition exposed the employees to
a struck-by
hazard.
Among other methods, one feasible and acceptable method to correct this
hazardous
condition is to relocate the pipe to a place where it does not obstruct
the Geronimo line's
escape route in accordance with the American Petroleum Institute
Recommended Practice
for Occupational Safety for Oil and Gas Well Drilling and Servicing
Operations (API 54
6.10.1) that states: "The escape line route should be kept clear of
obstructions".
