a)At the Block Machine:  On or about December 28, 2011 and at times prior
employees were exposed to struck-by hazards while using a underhung hoist
without a safety latch.  The employer failed to ensure frequent
inspections had
been performed by an operator or other designated person of the hoist for
missing
or defective equipment.
A feasible and acceptable methods to correct this hazard is to conduct
frequent inspections
during operations of the hoist for missing or defective equipment in
accordance with The
American Society of Mechanical Engineers (ASME) B30.10-2009, Hooks.-2009,
