Condition:Employees were exposed to being crushed by collapsing/failing
boom since
recommended inspection/maintenance schedule, as established by the
manufacturer,
was not followed resulting in visible problems such as rusted controls,
structural
damage and excessive rust to the platform supports, missing platform
instructions and
warnings and lack of upper controls.
Among other methods, one feasible and acceptable method to correct this
hazard is to comply with the
manufacturer's instructions regarding established inspection and
maintenance schedules to assure that the aerial
lift equipment is maintained in safe operating condition and the most
recent edition of American National
Standard Institutes "Vehicle Mounted Elevating and Rotating Aerial
Devices" ANSI A 92.2, Section 8
requirements entitled "Responsibilities of Owners, Testing and
Inspection", Section 9: "Responsibilities of
Users, Inspection and Maintenance".
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
