Health Act of North Carolina:  The
employer did not furnish to each of his employees  conditions of
employment and a place of employment free
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees, in
that employees were exposed to fire and/or an explosion:
a)Duke Hospital 6th floor - Where the employer failed to properly train
Code Team employees
(both permanent and contract) specifically on the potential fire hazards
of an oxygen-enriched
environment during the administration of defibrillation.  On November 6,
2012, at
approximately 0200 hours, during the administration of a fourth
defibrillation attempt on a
cardiac arrest patient, who was simultaneously being administered oxygen,
a fire resulted from
a spark from the defibrillator, which ignited excess oxygen during the
resuscitation attempts.
