Nevada Revised Statute 618.375(1): Duties of employers.  Every employer
shall furnish
employment and a place of employment which are free from recognized
hazards that are causing

employees.
At the Maverick Helicopters, Inc. workplace, employees were exposed to
fall hazards
when accessing the platform (5 feet 5 5/8 inches in height) of a
helicopter.  Employees
were required to step on the top step and top of 4 foot fiberglass
stepladders (make:
Werner) in order to access the platform to perform maintenance work on
helicopters.
Employees were exposed to serious fall hazards when accessing the platform
of the
helicopter in a manner that was not approved by the manufacturer.

9.3.2 Climbing and Working Locations. The user shall climb or work
with the body near the middle of the step of the rung. The user shall not
step or stand higher than the step or rung indicated on the label marking
the highest standing level of a ladder. The user shall not step or stand
on:
(1) A ladder top cap and the top step of a step or trestle ladder, or
the bucket or pail shelf on a self-supporting ladder.
One feasible and acceptable means of abatement method is to ensure that
employees
follow all safety rules on the proper use of ladders.s
