Equipment:Crown Forklift, Model #30SP42TT-S, Serial #H-7428-2
Condition:A propane powered forklift was not equipped with a seatbelt.
Among other methods, one feasible and acceptable method to correct this
hazard, install and
utilize a restraint system in accordance with the current version of
American National Standards
Institute (ANSI) B56.1, "Safety Standard for Powered Industrial Trucks,"
and American Society
of Mechanical Engineers (ASME) B56.1-2012, "Safety Standard for Low Lift
and High Lift
Trucks."
Abatement documentation (photographs, receipts, copies of programs,
training
certification, etc.) is required for this item.

40 PVC Pipe
Condition:Schedule 40 polyvinyl chloride plastic pipe was utilized for
conveying compressed air
at pressures of 110 pounds per square inch from the air receiver to
various locations
throughout the facility.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to replace the
PVC pipe with air lines approved for delivering compressed air.
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
