The employees were wearing full body harnesses but were not able to utilize an active conventional fall protection system because the roof anchors had not been installed. The condition exposed employee to a 13' 5" fall hazard on the north side of the roof.

29 CFR 1926.502(d)(16)(iii):  Personal fall arrest systems, when stopping a fall, were not rigged such that employees can neither free fall more than 6 feet, nor contact any lower level:    (a) Douglass Colony Group, Inc. @ 1900 Longs Peak Ave., Longmont, CO 80501:  On and before May 24, 2011, the exposing employer did not ensure that an employee, who was engaged in roofing operations on the roof of the Sunset Bath House, used a personal fall arrest system that limited fall distances to 6 feet, or which prevented contact with the lower level.  The employee was using a rope grab fall protection system and did not adjust the rope grab cam lock device on the rope to prevent a free fall of more than 6 feet or resulting in a pendulum effect,  if a fall event occurred. The condition resulted in an employee exposed to a free fall greater than 6 feet.

29 CFR 1926.1053(b)(4):  Ladders were used for purposes other than the purposes for which they were designed:    (a) Douglass Colony Group, Inc. @ 1900 Longs Peak Ave., Longmont, CO 80501: On and before May 24th, 2011, the exposing employer allowed employees to utilize a Werner 14' A-frame ladder leaned against the fascia to obtain access to the roof.  The specific use of not opening and locking the spreaders while using the ladder is contrary to the manufacturers instuctions. This condition exposed employee's to a fall of approximately 11 feet.
