29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On June 27, 2011, during roofing activities, the employer allowed employees to work under other employees who was working on a roof above them without head protection, exposing the employee to possible head injuries.

29 CFR 1926.502(d)(15):     Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.    On June 27, 2011, during roofing activities, the employer allowed 3 to 4 employees to use the same personal fall arrest anchor designed to support only one employee. The employees were working on a roof that was approximately 18.4 feet high.
