29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. (a) Wolverine Enterprises, LLC, East Plateau Site @ My Way Ranch 17-2, Collbran, CO 81624: On or before June 29, 2011, the employer did not ensure the control of ignition sources while employees power washed the inside of storage tanks containing flammable vapors. On June 29th, 2011, two employees were seriously injured after the atmosphere inside the 300 barrel storage tank exploded.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Wolverine Enterprises, LLC, East Plateau Site @ My Way Ranch 17-2, Collbran, CO: On or before June 29, 2011, the employer did not ensure procedures were developed, documented and utilized for control of potentially hazardous energy including but not limited to internal electric tank heaters and 4 inch ventilation lines interconnecting the tanks prior to cleaning 2-400 barrel and 1-300 barrel storage tanks, . On June 29th, 2011, two employees power washing the inside of a 300 barrel storage tank were seriously injured after the hazardous atmosphere ignited.
