29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a. In the Tool Room an employee was exposed to the unguarded portion of the saw blade on the Roll-In Bandsaw Machine, approximately four inches above the blade guide rollers, while operating the bandsaw.    b. In the Production Area an employee was exposed to a one inch gap between the upper die and punches during operation of the Bantam Press Brake as a Punch Press.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards,shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    In the Production Area, CNC Machining Room, an employee's hand was exposed to a crushing hazard from the lack of point of operation guarding on the pneumatically operated "Bender" machine.

29 CFR 1910.212(a)(5):     Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above thefloor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch.    In the Production Area at the Three-way Station Thermoforming Machine, an employee was using a white plastic pedestal fan to assist in the cooling off of a form coming out of the oven which was missing one-half of its front blade guard.

29 CFR 1910.133(a)(3):     The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.    In the Production Area an employee was exposed to the flying plastic chips from the operation of a 5-Axis Router Machine by wearing prescription eye glasses which did not incorporate the design in this eye protection.

In the Production Area an opening, approximately 10 inches square, had an expanded metal mesh guard on the upper wheel of the Bental Marge, Hamilton, Ohio Bandsaw Machine with jagged edges which created a hazard to an employee while making an adjustment to the working portion of the saw blade.
