29 CFR 1926.453(b)(2)(v):     A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    An employee was exposed to a fall hazard up to approximately 18 feet to the ground below while operating an aerial lift to cut kerfs in the brick to install counter flashing and the employee did not wear a body belt or lanyard attached to the basket.
