29 CFR 1926.102(a)(1):  Eye and face protective equipment was not provided when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    a)  John Sheldon dba J & S Siding, Bloomdale, Ohio:  On or about July 15, 2011, at address 822 W. Bigelow, Findlay, OH, the employer failed to ensure that each employee was provided appropriate eye protection while engaged in nailing activities, such as securing shingles to the structure roof with a pneumatic nailer, exposing employees to eye injuries from flying particles.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    a)  John Sheldon dba J & S Siding, Bloomdale, Ohio:  On or about July 15, 2011, at address 822 W. Bigelow, Findlay, OH, the employer did not ensure that employees were protected from falls while applying new roofing to a sloped roof on a residential type structure.  Employees were exposed falls of approximately 15 ft. to the ground.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  John Sheldon dba J & S Siding, Bloomdale, Ohio:  On or about July 15, 2011, at address 822 W. Bigelow, Findlay, OH, the employer did not ensure that the side rails of an extension ladder were extended at least three feet above the landing.  Employees were exposed to falls while using the ladder to access the working level to perform roofing activities.
