29 CFR 1910.36(d) (1): Employees were unable to open an exit route door from the inside at all times without keys, tools, or special knowledge.      a) On or about July 27, 2011, at the above located workplace employees were required to work on the fourth floor. The rear emergency exit door on the fourth floor could not be opened without using a rubber mallet. The employees did not have a readily available emergency exit and thereby were exposed to the hazards associated with fires and smoke inhalation.
