29 CFR 1910.119(d)(3)(ii): Equipment did not comply with recognized and generally accepted good engineering practices: (a) In the DU1 unit at the P-3683 suction pump, the one inch three-hundred pound seal flush auxiliary piping line was made up with threaded connections and improperly fabricated pipe nipples. The configuration subrogated fabrication specifications as prescribed in American Petroleum Institute Standard 610, Centrifugal Pumps for Petroleum, Petrochemical and Natural Gas Industries, Section 7.5 Piping and Appurtenances. Corrected during inspection.

(b) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 6 - All furnace passes fail open. (c) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 8 - Loss of flow through an exchanger. (d) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 11 - Exchanger inadvertently bypassed. (e) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 16 - Loss of steam. (f) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 17 - Loss of electricity. 29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process: (a) There was no MOC (management of change) conducted following a 7/8/11 process flash crude release in the DU1 unit at the P3683 suction pump one inch three hundred pound seal flush piping which precipitated subsequent line replacement. The threaded components of the auxiliary process liquid piping were replaced with a different alloy tubing material with expansion loops while a flange and orifice assembly, dial gauge, and strainer were removed from the circuit. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
