At the work site located at 1525 Georgesville Road, Columbus, Ohio, the employer had two sets of scaffolding in bay 6 that were used by employees for servicing trailers. The scaffolding had bent braces and was missing a portion of one support leg. It was not taken out of service or repaired.

At the work site located at 1525 Georgesville Road, Columbus, Ohio, the employer had two sets of scaffolding in bay 6 that were used by employees for servicing trailers. The scaffolding had planking that had not been secured to prevent movement while in use. The locking mechanisms on the scaffolding were in place but had not been engaged. The planking was approximately twelve feet from the concrete floor.
