29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:    a)  On or about July 6, 2011, employees were exposed to injury while performing work on the drive mechanism of the North Scrap Crane in the Steelmaking area where the power was not shut down to the crane prior to performing this work.  This further resulted in the lack of implementation of remaining applicable energy control elements including machine isolation - 29 CFR 1910.147(d)(3)), LOTO device application - 29 CFR 1910.147(d)(4), dissipation of residual energy - 29 CFR 1910.147(d)(5), verification of isolation - 29 CFR 1910.147(d)(6), and notification of affected employees 29 CFR 1910.147(c)(9).
