29 CFR 1910.255(b)(4): All press welding machine operations, where there was a possibility of the operator's fingers being under the point of operation, were not effectively guarded by use of a device such as an electronic eye safety circuit, two hand controls or protection similar to that prescribed for punch press operation,  1910.217 of this part:  (a)  On or about July 27, 2011, and at times prior, employees were exposed to injury, while operating the ACME spot welder which did not have a point of operation guard to protect the operators fingers from coming in contact with the electrodes, when performing spot welding operations.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):  (a) On or about July 27, 2011, and at times prior, the Superior Electric router located in the wood shop, had a pulley located on the side of the machine that was not guarded.
