29 CFR 1910.305(a)(2)(i):  Temporary electrical power and lighting installations rated 600 volts, nominal, or less were used for purposes other than those permitted in subparagraphs (A), (B) and (C) of this paragraph:    Throughout facility: On 07/28/11 and at times prior thereto, Several Patton Mach 30 industrial fans were powered with extension cords for a period exceeding 2 months. 29 CFR 1910.305(g)(2)(iii):  Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:      (a)Forklift charging station: On 07/28/11 and at times prior thereto, 36 vdc forklift charging cable end fittings were not fitted with strain relief devices.    Forklift charging station: On 07/28/11 and at times prior thereto, the flexible power cable was pulled loose from the 120 vac female end fitting on a Hertner Auto 6000, 36v DC, forklift battery charger, locally identified and item CH36-10.    Note: Abatement certification IS NOT required for this item.
