29 CFR 1910.138(a): General requirements. Employers shall select and require employees to use appropriate hand protection when employeess' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.     Production area - On or about August 3, 2001, employees were allowed to operate four vertical Bird meat cutting band saws without the proper hand protection. Employer did not select the appropriate hand protection in the form of cut resistant gloves relative to the tasks being performed. Employees wore cloth gloves while cutting/trimming meats. Employees were thereby exposed to the hazards associated with rotating saw blades.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy controlprocedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine orequipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.     Entiire Facillty - On or about August 3, 2011, a written energy control program (lockout/tagout) consisting of specific procedures and training was not available for employees exposed to the hazards of moving parts when performing service and maintenance on the Koch meat grinder, Grassell mixer/blender and  operating equipment.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with amaximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.    Maintenance Shop - On or about August 3, 2011, the Dayton pedestal grinder did not have the work rest for the right or left side of the grinding wheels, thereby exposing employees to the hazards of flying particles in the event the grinding wheel shattered.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of thisparagraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to theconstantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs(b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of theperipheral member at the top shall never exceed one-fourth inch.     Maintenance shop - On or about August 3, 2011, the pedestal grinder did not have a tongue guard for the right or left sides of the grinding wheels, thereby exposing employees to the hazards of flying particles in the event the grinding wheel shattered.
