On or about January 29, 2008, a recordable injury occurred to an employee resulting in tire fell on top + crushed legs. The description in Column F did not describe the injury or illness or the object/substance that directly injured the employee.    b. On or about March 19, 2008, a recordable injury occurred to an employee resulting in dropped part on foot. The description in Column F did not describe the injury or illness or the object/substance that directly injured the employee.    c. On or about May 23, 2008, a recordable injury occurred to an employee resulting in struck by lightening. The description in Column F did not describe the injury or illness or the object/substance that directly injured the employee.    d. On or about September 23, 2008, a recordable injury occurred to an employee resulting in support chain broke, fingers. On or about January 15, 2009, a recordable injury occurred to an employee resulting in strained rib closing gate.. The description in Column F did not describe the injury or illness or the object/substance that directly injured the employee.    b. On or about March 28, 2009, a recordable injury occurred to an employee resulting in bumped head on steel. The description in Column F did not describe the injury or illness or the object/substance that directly injured the employee.    c. On or about April 4, 2009, a recordable injury occurred to an employee resulting in strained back tugging.
