29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA.    a)  On or about August 5, 2011, employees were exposed to struck-by hazards from vehicular traffic while working and operating a forklift in live lanes of traffic without traffic controls in place to alert and direct motorists safely around the work area.  Lack of controls included no advance warning signs in place, no traffic control devices (cones, drums) to divert traffic, no trained traffic control supervisor onsite or a traffic control plan for this particular work.

29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition):    a)  On or about August 5, 2011, employees were exposed to struck-by hazards while working in live lanes of traffic where employees were directing live traffic without high visibility apparel or flagging devices (paddles).  Employees were also not trained on flagger safety.
