29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment have been accomplished.    a. Jacobs Industrial Services, Inc. worksite located at 1900 Fort Amanda Road, Lima, Ohio:  On or about July 14, 2011, the employer did not ensure maintenance employees performing work on the spare steam reboiler located in the purification section of the facility, followed the Ineos lockout program which stated field verification of lockout points must be performed by all authorized employees with the Ineos operator.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    a. Jacobs Industrial Services, Inc. worksite located at 1900 Fort Amanda Road, Lima, Ohio:  On or about July 14, 2011, the employer did not ensure employees tasked with conducting annual audits of the company's lockout program, determined if field verifications were being performed with all authorized employees.    Auditors verified that filed verifications were being performed the first day of the project between foreman and the employees who were present at the time but failed to ensure field verifications were being performed with employees arriving to perform work on subsequent days.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
