WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.178(P)(1) WHICH WAS CONTAINED IN OSHA INSPECTION 311373369, CITATION NUMBER (1), ITEM (001), AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 13, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 901 CARLOW DRIVE BOLINGBROOK, IL 60490.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.    a. Warehouse - On or about August 25, 2011, employees were required to operate material handling equipment and defects on the trucks were not reported and corrected. The defects found were the following;    1. The right load wheel on the (Raymond) electric pallet jack # 8 was missing a piece of its rubber.  2.  The cover for the battery box on the (Raymond) electric pallet jack # 19 was loose.  3.  The plastic cover for the pop off speed function button on the (Raymond) electric pallet jack # 27 was missing.  4.  The load back rest on the (Toyota) fork lift truck # 4 was dented.
