29 CFR 1910.244(a)(2)(iii): The load, which was raised by a jack, was not cribbed, blocked, or otherwise secured:    On or about August 19, 2011, at the above addressed jobsite, an employee received fatal head injuries when a lawn mower fell from a hydraulic jack.  The lawn mower was not cribbed or blocked when it was raised.
