29 CFR 1910.178(a)(4):     Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturer's written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly:    a)  On 08/24/2011, a Caterpillar T1200 forklift in the shipping/receiving area had additions of counterweights on the rear of the vehicle which were added without the manufacturer's written approval and the capacity, operation and maintenance instruction plates were not changed accordingly.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.
