(c)  Employees were not instructed in the use of the hoist and in the steps of performing an assessment of materials to be lifted to ensure capacity of the hoist is not exceeded.    (d) The conveyor attached to the hoist was left in a suspended postion without implementing specific precautions to prevent collapse in the event of hoist failure.    (e) The control actuator of the hoist control did not indicate the direction of the hoist movement.

The wiring connected to the hoist was made up of two seperate electrical cords that were spliced together with electrical tape.    (f) Electrical outlet box mounted on west side of structural column located between the Williams one (1) and Williams two (2) tire grinder was damaged exposing the outlet wiring. This exposed employees to an electrical shock hazard.    (g) Williams 2 Grinder: Electrical wires wired into the magnetic belt outfeed conveyor motor were exposed, in-that the motor wire connection housing cover was removed. This exposed employees to an electrical shock.
