29 CFR 1926.550(a)(5):     The employer shall designate a competent person who shall inspect all machinery and equipment priorto each use, and during use, to make sure it is in safe operating condition. Any deficiencies shall be repaired, or defective parts replaced, beforecontinued use.    Workers were engaged in commercial construction activities building a new pole type of structure. Workers were exposed to potential fall, struck by, and head injury hazards in that the truck mounted, 1974 COMTRUCK, M-TC 504B, S-981 crane was not properly inspected by a competent person.

29 CFR 1926.404(f)(6):     The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    Located on the exterior and interior of the structure there was a DeWalt circular saw connected to the electrical service with two 100.0 foot black extension cords. Worker(s) were exposed to potential electrocution hazards in that there was a missing grounding pin on the plug.

29 CFR 1926.501(b)(1):     "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrailsystems, safety net systems, or personal fall arrest systems.    Workers were observed engaged in commercial construction activities building a new pole barn type of structure. Workers were installing wooden framing material on the upper truss area exposed to potential fall hazards from heights between 15.0 and 26.0 feet in that a method of convential fall protection system was not utilized.

Workers who were exposed to fall hazards were not properly trained in fall protection criteria.
