29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. a) At the Network Global Logistics facility located in Lockborne, Ohio forklift inspections that were completed on a daily basis had found defects with the forklifts, documented the defects on the employer's inspection form, and the defects were not corrected.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a) At the Network Global Logistics facility, located in Lockborne, Ohio the Yale forklift #2 (Serial # C184N02490F) had a light on it that had broken off, and the lift was not removed from service. b) At the Network Global Logistics facility, located in Lockborne, Ohio the Yale forklift #10 (Serial # C814N02056F) had a light on it that had broken off, and the lift was not removed from service. c) At the Network Global Logistics facility, located in Lockborne, Ohio the Yale forklift #4 (Serial # C814N02519F) had a light on it that had broken off, and the lift was not removed from service. d) At the Network Global Logistics facility, located in Lockborne, Ohio the Yale forklift #8 (Serial # C814N02500F) had a light on it that no longer worked, and the lift was not removed from service.

29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service. a) At the Network Global Logistics facility located in Lockborne, Ohio the employer had a forklift attachment located at the battery charging area that used a sling and hook to change batteries back and forth from the forklifts to the charging area. The chain link sling had a hook attachment that had a broken safety latch, and was not removed from service.

29 CFR 1910.37(b)(6): Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m��?) are permitted. a) At the Network Global Logistics facility, the Exit sign between overhead doors 43 and 44 was not illuminated because the bulbs were burnt out. b) At the Network Global Logistics facility, the Exit sign between overhead doors 10 and 11 was not illuminated because the bulbs were burnt out. c) At the Network Global Logistics facility, the Exit sign between overhead doors 49 and 50 was not illuminated because the bulbs were burnt out. d) At the Network Global Logistics facility, the Exit sign between overhead doors 55 and 56 was not illuminated because the bulbs were burnt out. e) At the Network Global Logistics facility, the Exit sign over the exit door by the battery charging area was not illuminated because the bulbs were burnt out. f) At the Network Global Logistics facility, the Exit sign between by the Kroger office was not illuminated because the bulbs were burnt out.
