29 CFR 1910.132(d)(1)(i):     When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard:     On or about July 26, 2011, an employee was not protected from falling approximately 9 feet 8 inches while working on a metal platform on top of a tank trailer.  The employer provided a 6 foot lanyard with a shock absorber device, which when fully deployed would extend approximately 11 feet in length.  The employer instructed the affected employee to attach the lanyard at a location below 10 feet from a lower level.
