29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged: (a) JD Field Services Inc., at Stoddard 33-30, Craig, CO 81625: On August 10, 2011, the employer did not ensure the load was stable when lifted and being moved on the Gehl Model DL12 forklift in that the spreader was moved on the forklift without being arranged or strapped onto the forks to ensure its stability. Because of this conduction, a spreader fell off the forks of the forklift presenting a struck-by hazard to employees.
