29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.  (a)  Inadequate fall protection, three rows of roof jacks, was utilized while performing roofing activities exposing employees to fall hazards.  THE TOTAL COVERAGE ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313528192, CITATION NUMBER 1,  ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 2, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 3834 E. LAYTON AVE., CUDAHY, WI 53110.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  The extension ladder used to access the roof did not extend at least three feet past the landing exposing employees to trip and/or fall hazards.  THE TOTAL COVERAGE ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.1053(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313528192, CITATION NUMBER 1,  ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 2, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 3834 E. LAYTON AVE., CUDAHY, WI 53110.
