29 CFR 1910.23(d)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standards railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)9i) through (v)), on all open sides except where there is entrance to a ramp, stairway, or fixed ladder:      (a)  Power Engineering Company at 2525 S Delaware St., Denver, CO 80223: On and before 9/6/11, the employer did not ensure that open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)9i) through (v)), on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The three air exchangers with open sided platform, eight feet above floor level, was not guarded by standard railings or the equivalent. This condition exposed employees to a fall hazard.    (b)  Power Engineering Company at 2525 S Delaware St., Denver, CO 80223: On and before 9/6/11, the employer did not ensure that open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)9i) through (v)), on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The stairway access from the back door of the maintenance shop, was not guarded by standard railings or the equivalent. This condition exposed employees to a fall hazard.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      Power Engineering Company at 2525 S Delaware St., Denver, CO 80223: On and before 9/6/11, the employer did not protect employees from hazards created by operating a band saw with an unused portion of the blade unguarded, an unguarded trash compactor and a rotating shaft of a table saw motor. This condition exposed employees to amputation and crush hazards.      1)	Trash compactor  2)	Blue vertical band saw  3)             Rotating shaft on the table saw motor    Abatement Note: A feasible and acceptable abatement method to correct the hazard includes compliance with the American National Standards Institute, ANSI Z245.2, which requires an operating interlock on door, also an inspection of the trash compactor to ensure the interlock is functioning, and not permit an employee to operate the equipment unless the interlock and gate enclosure is functioning.

29 CFR 1910.305(b)(2)(i):  All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy:     (a)  Power Engineering Company at 2525 S Delaware St., Denver, CO 80223: On and before 9/6/11, the employer did not protect employees from hazards created by uncovered junction box through which conductors enter. This condition exposed employees to electrical hazards.

29 CFR 1910.303(g)(1):  Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance od such equipment:    (a)  Power Engineering Company at 2525 S Delaware St., Denver, CO 80223: On and before 9/6/11, the employer did ensure that working space about electric equipment rated 600 volts, nominal, or less had sufficient access to the electrical service panel. This condition exposed employees to electrical hazards.
