29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):    a). On or about July 29, 2011, and at times prior,  the employer did not ensure employees wearing 1/2 face negative pressure respirators were fit-tested prior to performing painting operations.      Abatement Note:  Abatement certification is required for this item.

29 CFR 1910.134(g)(1)(i)(A):  The employer permitted respirators with tight-fitting facepieces to be worn by employees who had facial hair that came between the sealing surfaces of the facepiece and the face or that interfered with valve function:    a).  On or about July 29, 2011, and at times prior, employees wearing 1/2 face negative pressure respirators during painting operations had facial hair that could interfere with the proper function of the respirator.    Abatement Note:  Abatement certification is required for this item.
