29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a) At the establishment, conduit containing electrical wires that provided electrical power to the #7 Lennox air handling unit had broken at the electrical box placing strain on the electrical wires and terminal screws and exposing employees to an electrical shock hazard.     b) At the establishment, conduit containing electrical wires that provided electrical power to the #4 Lennox air handling unit had broken at an elbow placing strain on the electrical wires and terminal screws and exposing employees to an electrical shock hazard.

29 CFR 1904.29(b)(1):     What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.    a) At the establishment, column "F" of the OSHA-300 for 2009 did not include all the information required for the 6 recorded injuries and illnesses. Column "F" must include a description of the injury, the parts of the body affected, and the object/substance that directly injured or made employee ill.
