29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    Eric Galinski, conducting work at 27511 Holiday Lane, Perrysburg, OH: On or about September 12, 2011, the employer failed to ensure that employees, operating a JLG aerial lift (Model 660SJ, Serial # 0300087106), were adequately protected from fall hazards.  The exposed employees were not tied off to the basket of the aerial lift while it was in use. Employees were exposed to approximately 25 feet fall hazard to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.454(a):     The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    Eric Galinski, conducting work at 27511 Holiday Lane, Perrysburg, OH: On or about September 12, 2011, the employer did not ensure that employees, operating a JLG aerial lift (Model 660SJ, Serial # 0300087106), were trained by a person, who is qualified in the subject matter, to recognize the hazards associated with the type of the aerial lift being used and to understand the procedures to control or minimize those hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:    Eric Galinski, conducting work at 27511 Holiday Lane, Perrysburg, OH: On or about September 12, 2011, the employer did not ensure that an employee was protected from fall hazards while performing roofing work on a low-slope roof of the commercial building. The exposed employee was caulking the roof in preparation for painting. The employee, engaged in this activity, was exposed to approximately 25 feet fall hazard from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Eric Galinski, conducting work at 27511 Holiday Lane, Perrysburg, OH: On or about September 12, 2011, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while performing roofing work on a low-slope roof of the commercial building. Employees did not use any form of fall protection and were exposed to approximately 25 feet fall hazard from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
