29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Eric Galinski, conducting work at 27511 Holiday Lane, Perrysburg, OH: On or about September 12, 2011, the employer failed to ensure that employees, operating a JLG aerial lift (Model 660SJ, Serial # 0300087106), were adequately protected from fall hazards. The exposed employees were not tied off to the basket of the aerial lift while it was in use. Employees were exposed to approximately 25 feet fall hazard to the ground below. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Eric Galinski, conducting work at 27511 Holiday Lane, Perrysburg, OH: On or about September 12, 2011, the employer did not ensure that employees, operating a JLG aerial lift (Model 660SJ, Serial # 0300087106), were trained by a person, who is qualified in the subject matter, to recognize the hazards associated with the type of the aerial lift being used and to understand the procedures to control or minimize those hazards. The exposed employee was caulking the roof in preparation for painting. The employee, engaged in this activity, was exposed to approximately 25 feet fall hazard from the roof to the ground below. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees did not use any form of fall protection and were exposed to approximately 25 feet fall hazard from the roof to the ground below.
