CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     	(a)	Precision Metal, 11060 Irma Drive, North Glenn, CO 80234:  On and before September 21, 2011, the controlling and exposing employer did not ensure that all affected employees utilizing a Sharp Drill Press, Serial Number 60200326, which had modifications made to the left side (as one faces the machine) of the press to allow the bit to be raised and lowered, was guarded from hazards created by the machines rotating parts and flying chips and sparks. This condition exposed employees to hazards from the machines rotating parts and hazards from chips and/or sparks while utilizing the press in the machine shop.     Abatement Note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Worksheet").

CFR 1910.303(b)(2): Listed or labeled electrical equipment was not installed and used in accordance with any instructions included in the listing or labeling.    	(a)	Precision Metal Manufacturing, Inc., 11060 Irma Drive, North Glenn, CO 80234:  On and before September 21, 2011, the controlling and exposing employer did not ensure that electrical equipment was installed and used in accordance with any instructions included in the listing or labeling.  Specifically, electrical extension cords were seen connected to junction boxes located in the ceiling; cords were then wrapped around metal ceiling joists and vertically dropped to provide electrical power to various machines.  Condition exposed employees to electrical hazards while utilizing the equipment.       Abatement Note:  Abatement certification is required for this item (See enclosed Sample Abatement-Certification Worksheet).
