Hopkins, Aspen, CO 81611:  On September 27, 2011 as the exposing employer, did not ensure the employees were protected by the use of a fall protection system meeting the requirements of 1926.502 of this Subpart in that employees who conducted carpentry activities on the 2nd floor were not protected by a fall protection system installed in all openings in the exterior walls.  This condition exposed employees to the hazard of a fall of up to 12 feet.

Hopkins, Aspen, CO 81611:  On September 27, 2011, as the exposing employer, did not ensure that employees were protected from stepping into or through holes by covers. Employees were exposed to a falling hazard due to an open floor hole located within 2 feet of the upper landing area associated with a portable ladder that was used to access the 3rd floor. The floor hole measured approximately 4 feet long and13.5 inches wide. This condition exposed employees to a fall hazard as it was located along a common path of travel.
