29 CFR 1904.29(b)(1):     A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation.    a)  The OSHA 300 log for calendar year 2007 contained recording errors in that column F did not describe the injury or completely describe the part of the body related to events.    b)  The OSHA 300 log for calendar year 2008 contained recording errors in that column F did not describe the injury or completely describe the part of the body related to events.    c)  The OSHA 300 log for calendar year 2009 contained recording errors in that column F did not describe the injury or completely describe the part of the body related to events.    d)  The OSHA 300 log for calendar year 2010 contained recording errors in that column F did not describe the injury or completely describe the part of the body related to events.    e)  The OSHA 300 log for partial clendar year 2011 contained recording errors in that column F did not completely describe the part of the body related to events.  On entry #89 restrictions are indicated in column I but days of restriction are not listed in column K.

29 CFR 1910.1030(f)(1)(ii)(A):     The employer did not ensure that medical evaluations and procedures including the hepatitis B vaccine and vaccination series, post-exposure evaluation, or follow-up, including prophylaxis, were made available at no cost to the employee:    (a)  The company did not provide an employee the hepatitis B vaccination and titer without cost that was allowed to receive the series from their own physician.
