29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:    a) On or about September 20, 2011 and times prior, employees were exposed to injuries from falls in that, they were working in an elevated manbasket on the forks of a lift without being appropriately restrained within the basket, and with the gate of the basket left open.    b) On or about September 20, 2011 and times prior, employees were exposed to injuries from falls greater than 6 feet when installing siding material from a ladder jack scafford platform.    Abatement Verification:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(13):  The top or top step of a stepladder was used as a step:    a) On or about September 20, 2011 and times prior, employees were exposed to injuries from falls when installing a timber from a stepladder. An employee was standing on the top step installing a timber support. The ladder used by the employee was located on the south patio deck.
