29 CFR 1926.100(a):   Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) At the Glorioso's jobsite; hard hats were not worn by the employee working on the ground level with an employee above working on a gutter system.  This exposed the employee to struck by hazards.

29 CFR 1926.451(g)(1)(i):   Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a)  At the Glorioso's jobsite; fall protection was not utilized while working from a ladder jack to install a gutter system.  This exposed an employee to approximately 17.8' fall hazards.
