29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person  (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):                                                                        Employees conducting roofing work on an approximately 7 in 12 pitch roof were exposed to an approximately 9 foot fall and the employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      The employer's training program did not enable each employee to recognize the hazards of falling and was insufficient to train each employee in procedures to be followed in order to minimize fall hazards while employees worked on an 9 foot high roof which had an approximate 7 in 12 pitch.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    Employees were exposed to a fall hazard of approximately 9 feet to the ground below while performing roofing work on the top of approximately 7 in 12 pitch roof.
