29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above the lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems nor was another provision in paragraph (b) of this section provided:    a)  Juan Miramontes, Findlay, Ohio:    On or about October 14, 2011, at address 3326 Ridgeview Dr., Findlay, Ohio, the employer did not ensure that employees were protected from falls while removing old roofing material from a low sloped roof (4:12) on a residential type structure.  Employees were exposed to a potential fall of approximately 18.3 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    a) Juan Miramontes, Findlay, Ohio:  On or about October 14, 2011, at address 3326 Ridgeview Dr., Findlay, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection systems while performing various roofing operations.  Employees were exposed to falls of approximately 18.3 feet to the ground below, which could result in permanent disability or death.

29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access to upper landing surfaces did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used, nor was a grasping device provided:    a) Juan Miramontes, Findlay, Ohio:  On or about October 14, 2011, at address 3326 Ridgeview Dr., Findlay, Ohio, the employer did not ensure that the side rails of an extension ladder were extended at least three feet above the landing.  Employees were exposed to falls while using the ladder to access the working level to perform roofing activities.
