Section 5(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while working from a work platform on a rough terrain forklift that exceeded an overall width greater than 10 inches on either side of the load bearing wheels:    (a)  On or about October 5, 2011, and at times prior to that date, for the employees installing siding while working from a 15-foot forklift platform elevated by a Skytrac rough terrain forklift, with a 24-foot aluminum scaffold pick attached to the platform.  The employees were exposed to fall hazards of approximately 15 feet, located at 301 South Spring Avenue, Sioux Falls, South Dakota.
