29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet (0.6 m) above or below a point of access:    a) Employees were not provided with safe access to scaffolding from which they were required to work at an elevation of approximately 35 feet.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    a) Employees were exposed to the hazard of falling from elevation of approximately 15 and 35 feet while working from scaffolding without the benefit of fall protection.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.      Casey Tuckpointing, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451 (g)(1) which was contained in OSHA inspection number 314965120, citation 1, item 1b, issued on December 14, 2010, and became final order on or about January 6, 2011.
