29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Employees engaged in roofing operations were exposed to fall hazards of approximately 9ft because they accessed the roof via portable aluminum extension ladders and the ladder side rails had not been extended at least three feet above the roof surface being accessed.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.    Employees engaged in roofing operations were exposed to fall hazards due to ladders that were not extended 3ft above the roof surface and the employer had not trained the employees to recognize the hazards associated with ladders and how to minimize those hazards.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    Employees engaged in roofing operations on a residential structure at Chaddock school with a roof pitch of approximately 5/12 were exposed to fall hazards of up to 9ft and the employer did not provide any type of fall protection or fall protection plan.
