29 CFR 1904.32(a)(3): A company executive did not certify an OSHA 300A Form or equivalent:     a) On or about October 27th, 2011, the employer failed to certify the annual summary of work-related injuries and illnesses for calendar year 2009 by having the highest management official within the establishment sign the form.     b) On or about October 27th, 2011, the employer failed to certify the annual summary of work-related injuries and illnesses for calendar year 2010 by having the highest management official within the establishment sign the form.     No Abatement Certification or Abatement Documentation Required.
