29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system.    (a)Johnny Vasquez @ 4110 Coaldale Dr., Loveland, CO 80537: On and before October 1st, 2011, the exposing employer did not ensure employees were protected from injury, in that an employee engaged in roofing operations was exposed to fall hazards. This condition exposed the employee to a fall of approximately 12 feet.
