29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):    (a)Pipeline Industries Inc., at 2703 S. Tennyson Way, Denver, CO 80236:  On November 15, 2011 the employer did not ensure that each employee conducting work in an excavation was protected from cave-in, in that the west end of the excavation was approximately 8.2 feet deep and lacked an adequate protective system, such as appropriate sloping, benching, or shield system at the area the employee was working. The employee was exposed to the hazard in an area approximately 2 feet long and 2.5 feet wide and a tunnel approximately 1.5 feet into the end of the trench.  This condition exposed the employee to a crushing hazard by excavation wall and tunnel cave-in.    (b)Pipeline Industries Inc., at 2703 S. Tennyson Way, Denver, CO 80236:  On November 15, 2011 the employer did not ensure that each employee conducting work in an excavation was protected from cave-in, in that the east end of the excavation was approximately 10 feet deep and lacked an adequate protective system, such as appropriate sloping, benching, or shield system at the area the employee was working. The employee was exposed to the hazard in an area approximately 2.5 feet long and 2.5 feet wide and a tunnel approximately 2 feet into the end of the trench.  This condition exposed the employee to a crushing hazard by excavation wall and tunnel cave-in.
