29 CFR 1910.242(a):  Hand and portable powered tools or equipment were not kept in safe condition:    a) On or about October 27, 2011, and at times prior, Denbury Onshore, LLC, as controlling employer, did not ensure that Patterson-UTI Drilling Company LLC, a subcontractor, protected their employees from injury while using a pipe wrench where the lower set of jaws was missing. The employees used the wrench to connect and disconnect pipe.

29 CFR 1910.305(g)(2)(i):  Flexible cords or cables of the S, SC, SCE, SCT, SE, SEO, SEOO, SJ, SJE, SJEO, SJEOO, SJO, SJT, SJTO, SJTOO, SO, SOO, ST STO, STOO, G, G-GC, PPE and W type were not durably marked on the surface at intervals not exceeding 610 mm (24 in) with the type designation, size and number of conductors:    a) On or about October 27, 2011, and at times prior, Denbury Onshore LLC, as controlling employer, did not ensure that Patterson-UTI Drilling Company LLC, a subcontractor, protected their employees from injury in that, the rating on the flexible cord used to operate the washing machine could not be read.
