29 CFR 1910.23(a)(8)(ii):  A floor hole into which persons can accidentally walk was not guarded by a floor hole cover of standard strength and construction:    a) On or about October 30, 2011, and at times prior, employees were exposed to injury in that, half of the cellar floor cover was missing. The opening was 4 foot by 2 foot.    DHS Drilling Company, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.23(a)(8) which was contained in OSHA inspection 311905483 citation number 1 item 4 issued on 09/15/2008, and became a final order on 10/09/2008 with respect to a workplace located at Well #23-121, Colbran, CO 81624.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) On or about October 30, 2011, and times prior, employees were exposed to injury in that, the eye wash station located at the mixing area did not have any flushing fluid in it. Employees were using chemicals such as, but not limited to, Lime, a chemical known to be corrosive to the eyes.      DHS Drilling Company, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.151(c) which was contained in OSHA inspection 311905483 citation number 2 item 1 issued on 09/15/2008, and became a final order on 10/09/2008 with respect to a workplace located at Well #23-121, Colbran, CO 81624.
